Foster to Adopt Agreement

This agreement is between the Chunkz & Tubz Rescue Foundation (CTRF) and the above-named Foster Parent. CTRF agrees to give the foster parent possession and control of foster animals contingent upon the terms outlined below. As a foster parent, I agree to the following terms:

1. To provide food, shelter and care for the foster animal and assume all responsibility for the animal’s wellbeing.
2. To ensure that a dog/puppy lives primarily indoors and is NOT left outside unsupervised, even in a fenced yard, for extended periods. If the Foster Parent does not have a fenced yard, a dog/puppy will live inside at all times and be taken outside on a leash on a regular basis for exercise and elimination needs.
3. To comply with all state and local laws relative to the keeping and care of the animal.
4. To assume all responsibility for damage caused by the animal either directly or indirectly while in the Foster Parent’s direct control.
5. The Foster Parent understands and agrees that CTRF will take every precaution necessary to ensure that the animal(s) are in reasonably good health. However, CTRF cannot be held responsible/liable for any unforeseen health problem that may develop once the animal is in my care. If unforeseen problems arise, the Foster Parent is responsible for keeping the animal separated from any other resident pets (in cases of contagious and/or easily transmittable illnesses) and is also responsible for any medical needs of resident animals as a result of such.
6. In the event that the Foster Parent cannot continue to foster the animal(s), the Foster Parent will contact CTRF and will await the finding of a new foster home (2-4 weeks)
7. Understand that I am making a commitment to fostering an animal until he/she has received the necessary vetting/training to be deemed adoptable and that at such time as CTRF deems appropriate we shall make a long-term commitment to the dog/puppy via a permanent adoption arrangement.
8. To bring the animal(s) to the vet for vaccinations, spay/neuter surgery or any other medical treatments as deemed necessary. (All treatment MUST be pre-approved barring extreme emergency)
9. Hereby agree to indemnify and hold harmless CTRF, and its Board of Directors, from any and all liability arising out of or in consequence of, or injury sustained as a result of, any activity connected with volunteering for CTRF as a foster home.
10. Understand and agree to be responsible for arranging for the safe care of all foster animal(s) in the event of travel and agree to pay all related boarding expenses.
11. To carry sufficient homeowner’s/renter’s insurance to cover injury or damage caused by a foster pet and is aware of any breed restrictions that may be in place.
12. If I hereby decide I wish to keep the animal(s) in my care by officially adopting, I agree to pay the rehoming fee of $500 to CTRF. I understand that if I opt to officially adopt this animal(s) but after a certain amount of time realize that I do not wish to keep the animal(s), that they will be dutifully returned to CTRF.
13. To plan to sign an official adoption contract no later than .
    In cases where medical needs are present, time can be extended as necessary. Our goal is to be sure ALL our animals get a forever home. Any length of time beyond the above must be communicated and arranged with CTRF to prevent removal of the animal to facilitate adoption to different parties.

By signing this agreement, the Foster Parent agrees that he/she has read and understands the above and agrees to abide by all of its provisions. CTRF may reclaim the animal(s), if in their judgment the animal(s) are not being cared for properly, or if in CTRF’s sole discretion, it is determined that the Foster Parent has not complied with the terms of this agreement.
